To register for Energym Classes, simply read the Energym Handbook and then click below to download a copy of the Registration Forms. You can fill out the forms then
fax or mail them back to Energym.

Family Information Form
This form is to be completed by New Energym families as well as each new child(ren) participating in our programs.

This form needs to be completed in order to enroll your child(ren) in any of the Energym Programs.
